RESUMO

This study presents an advanced metaheuristic approach termed the Enhanced Gorilla Troops Optimizer (EGTO), which builds upon the Marine Predators Algorithm (MPA) to enhance the search capabilities of the Gorilla Troops Optimizer (GTO). Like numerous other metaheuristic algorithms, the GTO encounters difficulties in preserving convergence accuracy and stability, notably when tackling intricate and adaptable optimization problems, especially when compared to more advanced optimization techniques. Addressing these challenges and aiming for improved performance, this paper proposes the EGTO, integrating high and low-velocity ratios inspired by the MPA. The EGTO technique effectively balances exploration and exploitation phases, achieving impressive results by utilizing fewer parameters and operations. Evaluation on a diverse array of benchmark functions, comprising 23 established functions and ten complex ones from the CEC2019 benchmark, highlights its performance. Comparative analysis against established optimization techniques reveals EGTO's superiority, consistently outperforming its counterparts such as tuna swarm optimization, grey wolf optimizer, gradient based optimizer, artificial rabbits optimization algorithm, pelican optimization algorithm, Runge Kutta optimization algorithm (RUN), and original GTO algorithms across various test functions. Furthermore, EGTO's efficacy extends to addressing seven challenging engineering design problems, encompassing three-bar truss design, compression spring design, pressure vessel design, cantilever beam design, welded beam design, speed reducer design, and gear train design. The results showcase EGTO's robust convergence rate, its adeptness in locating local/global optima, and its supremacy over alternative methodologies explored.

RESUMO

Ramadan fasting (RF) involves abstaining from food and drink during daylight hours; it is obligatory for all healthy Muslims from the age of puberty. Although sick individuals are exempt from fasting, many will fast anyway. This article explores the impact of RF on individuals with kidney diseases through a comprehensive review of existing literature and consensus recommendations. This study was conducted by a multidisciplinary panel of experts.The recommendations aim to provide a structured approach to assess and manage fasting during Ramadan for patients with kidney diseases, empowering both healthcare providers and patients to make informed decisions while considering their unique circumstances.

There is a growing need for sensing materials that can provide multiple sensing capabilities for wearable devices, implantable sensors, and diagnostics tools. As complex human physiology requires materials that can simultaneously detect and respond to slow and fast pressure fluctuations. Mimicking the slow adaptive (SA) and fast adaptive (FA) mechanoreceptors in skin can lead to the development of dual sensing electrospun polymer nanocomposites for biomedical applications. These dual sensing nanocomposites can provide simultaneous sensing of both slow and fast pressure fluctuations, making them ideal for applications such as monitoring vital signs, detecting a wider range of movements and pressures. Here we develop a novel dual sensing PVDF-HFP-based nanocomposite that combines the advantages of capacitive and piezoelectric properties through controling electrospinning environment and processing parameters, polymer solution composition, and addition of nucleating agents such as Carbon Black (CB) to enhance the crystalline development of ß-phase, fibre thickness, and morphology. The developed PVDF-HFP/CB nanocomposite presents and response to both slow and fast pressure fluctuations with high capacitance (5.37 nF) and output voltage (1.51 V) allowing for accurate and reliable measurements.

Chronic kidney disease (CKD) is a major public health concern in the Middle East and Africa (MEA) region and a leading cause of death in patients with type 2 diabetes mellitus (T2DM) and hypertension. Early initiation of sodium-glucose cotransporter - 2 inhibitors (SGLT-2i) and proper sequencing with renin-angiotensin-aldosterone system inhibitors (RAASi) in these patients may result in better clinical outcomes due to their cardioprotective properties and complementary mechanisms of action. In this review, we present guideline-based consensus recommendations by experts from the MEA region, as practical algorithms for screening, early detection, nephrology referral, and treatment pathways for CKD management in patients with hypertension and diabetes mellitus. This study will help physicians take timely and appropriate actions to provide better care to patients with CKD or those at high risk of CKD.

This paper proposes a plan to manage energy consumption in residential areas using the demand response method, which allows electricity users to contribute to the reliability of the power system by controlling their usage. Due to the growing population, the residential sector consumes a significant amount of energy, and the objectives of this study are to lower electricity costs and the peak to average ratio, as well as reduce the amount of imported electricity from the grid. The study aims to maximize profit by properly utilizing renewable energy sources and addressing energy trading. The manta ray foraging optimization (MRFO) and long term memory MRFO (LMMRFO) algorithms are used to solve this problem. Firstly, the validation of the proposed LMMRFO technique is confirmed by seven benchmark functions and compared its results with the results of the well-known optimization algorithms including hunter prey optimization, gorilla troops optimizer, beluga whale optimization, and the original MRFO algorithm. Then, the performance of the LMMRFO is checked on the optimization of smart home energy management. In the suggested approach, a smart home decides whether to purchase or sell electricity from the commercial grid based on the cost, demand, and production of electricity from its own microgrid, which consists of a wind turbine and solar panels. Energy storage systems support the stable and dependable functioning of the power system since the solar panel and wind turbine only occasionally produce electricity. Through various case studies, the proposed plan is tested and found to be effective in reducing electricity costs and the peak to average ratio while maximizing profit. Furthermore, a comparative study is conducted to demonstrate the legality and effectiveness of LMMRFO and MRFO.

Metal-organic frameworks (MOFs) with intrinsic luminescent properties, modular structure, and tunable electronic properties, provide unique opportunities for designing target-specific molecular sensors by systematically choosing their constituent building blocks. We report a simple one-step MOF-based sensing platform for phosphate (P) detection that combines the luminescent properties of 2-aminoterephthalic acid (ATA) with the affinity of rationally selected nodes in UiO-66-NH2 to bind with P. This MOF possesses an electron-donating amine group that controls the light-harvesting characteristics of the linkers. Substituting Zr6 node with Ce6 or Hf6 results in a series of isostructural MOFs with distinct optical properties that are nonexistent in the unsubstituted MOF. We have utilized these MOFs to quantitatively measure P, using its ability to bind strongly to metal nodes inhibiting the LMCT process and altering the linker's photon emission. Using this system, detection limits of 4.5, 7.2 and 10.5 µM were obtained for the UiO-66-NH2(Ce), UiO-66-NH2, and UiO-66-NH2(Hf) respectively, adopting a straightforward single step procedure. These results demonstrate that the selection of metal nodes in a series of isostructural MOFs can be used to modulate their electronic properties and create sensing probes possessing the desired characteristics needed for the detection of environmental contaminants.

The supply-demand-based optimization (SDO) is among the recent stochastic approaches that have proven its capability in solving challenging engineering tasks. Owing to the non-linearity and complexity of the real-world IEEE optimal power flow (OPF) in modern power system issues and like the existing algorithms, the SDO optimizer necessitates some enhancement to satisfy the required OPF characteristics integrating hybrid wind and solar powers. Thus, a SDO variant namely leader supply-demand-based optimization (LSDO) is proposed in this research. The LSDO is suggested to improve the exploration based on the simultaneous crossover and mutation mechanisms and thereby reduce the probability of trapping in local optima. The LSDO effectiveness has been first tested on 23 benchmark functions and has been assessed through a comparison with well-regarded state-of-the-art competitors. Afterward, Three well-known constrained IEEE 30, 57, and 118-bus test systems incorporating both wind and solar power sources were investigated in order to authenticate the performance of the LSDO considering a constraint handling technique called superiority of feasible solutions (SF). The statistical outcomes reveal that the LSDO offers promising competitive results not only for its first version but also for the other competitors.

Subjective views of clozapine treatment among patients with schizophrenia in the Middle East and North African MENA Region have not previously been assessed. Globally, few studies have assessed the views of carers to clozapine treatment. We conducted a cross-sectional survey, using a clinician administered structured interview, of patients with schizophrenia/schizoaffective disorder prescribed clozapine in Qatar, and their primary carers. Participants were asked to rate clozapine against prior their antipsychotic treatment in terms of specific benefits and side effects. Forty-two patients and 33 carers participated in the study. Of the patients, two thirds were male, approximately half were Qatari and the mean age was 33.9 years. Patients and carers rated clozapine as superior to prior antipsychotic treatment on all 7 potential benefits inquired about. The greatest perceived benefit was improved mood. Patients rated clozapine as less likely to cause extrapyramidal side effects but more likely to cause 18 other potential side effects compared to prior antipsychotic treatment, with the greatest difference being for nocturnal salivation, increased appetite, and constipation. Nearly half of patients (48 %) and two thirds of carers (64 %) stated that they would have preferred to start clozapine earlier in their illness. Sixty percent of patients and 37 % of carers regarded the information that they had received from health professionals on clozapine as inadequate. Less than half of patients and approximately-two thirds of carers had adequate knowledge of haematological monitoring for clozapine. Generally, there were significantly positive correlation between patients and carers regarding the overall side effects of clozapine treatment. Likewise, improvement in hearing voices paranoid thoughts correlated with improved quality of life. In summary, the results show that patients and carers appreciate the benefits of clozapine despite its side effects being problematic. The results support clozapine being offered earlier in treatment and services providing more information on clozapine to patients and carers.

BACKGROUND: Psychotic experiences are reported in the general population. The Questionnaire for Psychotic Experiences (QPE) was created to test the phenomenological features of these experiences and compare them with those reported in patients with psychiatric and other medical conditions. The aim of this study was to test the psychometric properties of the Arabic version of the QPE. METHODS: We recruited 50 patients with psychotic disorders from the Hamad Medical Hospital in Doha, Qatar. Patients underwent assessment over three sessions with trained interviewees using the Arabic versions of QPE, Positive and Negative Syndrome Scale (PANSS), Beck Depression Inventory (BDI), and the Global Assessment of Functioning Scale (GAF). Patients were also reassessed using the QPE and GAF after 14-days from the initial assessment in order to test for the stability of the scale. In this respect, this is the first study that assesses the test-retest reliability of the QPE. The psychometric properties including convergent validity, stability, and internal consistency met the benchmarked criteria. RESULTS: Results confirmed that the Arabic version of QPE accurately measured the experiences of patients that were also reported using the PANSS, an internationally accepted, well-established scale for measuring psychotic symptom severity. CONCLUSION: We propose the use of the QPE to describe the phenomenology of PEs across modalities in Arabic speaking communities.

This research investigates the accelerated hydrolytic degradation process of both anatomically designed bone scaffolds with a pore size gradient and a rectangular shape (biomimetically designed scaffolds or bone bricks). The effect of material composition is investigated considering poly-ε-caprolactone (PCL) as the main scaffold material, reinforced with ceramics such as hydroxyapatite (HA), ß-tricalcium phosphate (TCP) and bioglass at a concentration of 20 wt%. In the case of rectangular scaffolds, the effect of pore size (200 µm, 300 µm and 500 µm) is also investigated. The degradation process (accelerated degradation) was investigated during a period of 5 days in a sodium hydroxide (NaOH) medium. Degraded bone bricks and rectangular scaffolds were measured each day to evaluate the weight loss of the samples, which were also morphologically, thermally, chemically and mechanically assessed. The results show that the PCL/bioglass bone brick scaffolds exhibited faster degradation kinetics in comparison with the PCL, PCL/HA and PCL/TCP bone bricks. Furthermore, the degradation kinetics of rectangular scaffolds increased by increasing the pore size from 500 µm to 200 µm. The results also indicate that, for the same material composition, bone bricks degrade slower compared with rectangular scaffolds. The scanning electron microscopy (SEM) images show that the degradation process was faster on the external regions of the bone brick scaffolds (600 µm pore size) compared with the internal regions (200 µm pore size). The thermal gravimetric analysis (TGA) results show that the ceramic concentration remained constant throughout the degradation process, while differential scanning calorimetry (DSC) results show that all scaffolds exhibited a reduction in crystallinity (Xc), enthalpy (Δm) and melting temperature (Tm) throughout the degradation process, while the glass transition temperature (Tg) slightly increased. Finally, the compression results show that the mechanical properties decreased during the degradation process, with PCL/bioglass bone bricks and rectangular scaffolds presenting higher mechanical properties with the same design in comparison with the other materials.

BACKGROUND: The Composite International Diagnostic Interview (CIDI) has been clinically reappraised in several studies conducted mainly in the US and Europe. This report describes the methodology used to conduct one of the Middle East's largest clinical reappraisal studies. The study was carried out in conjunction with the World Mental Health Qatar-the first national psychiatric epidemiological study of common mental disorders in the country. This study aimed to evaluate the diagnostic consistency of core modules of the newly translated and adapted Arabic version of the CIDI 5.0 against the independent clinical diagnoses based on the Structured Clinical Interview for DSM-5 (SCID-5). METHODS: Telephone follow-up interviews were administered by trained clinicians using the latest research edition of the SCID for DSM-5. Telephone administered interviews were key in the data collection, as the study took place during the COVID-19 pandemic. RESULTS: Overall, within 12 months, 485 interviews were completed. The response rate was 52%. Quality control monitoring documented excellent adherence of clinical interviews to the rating protocol. CONCLUSIONS: The overall methods used in this study proved to be efficient and effective. For future research, instrument cultural adaptation within the cultural context is highly recommended.

This paper proposes an enhanced version of Equilibrium Optimizer (EO) called (EEO) for solving global optimization and the optimal power flow (OPF) problems. The proposed EEO algorithm includes a new performance reinforcement strategy with the Lévy Flight mechanism. The algorithm addresses the shortcomings of the original Equilibrium Optimizer (EO) and aims to provide better solutions (than those provided by EO) to global optimization problems, especially OPF problems. The proposed EEO efficiency was confirmed by comparing its results on the ten functions of the CEC'20 test suite, to those of other algorithms, including high-performance algorithms, i.e., CMA-ES, IMODE, AGSK and LSHADE_cnEpSin. Moreover, the statistical significance of these results was validated by the Wilcoxon's rank-sum test. After that, the proposed EEO was applied to solve the the OPF problem. The OPF is formulated as a nonlinear optimization problem with conflicting objectives and subjected to both equality and inequality constraints. The performance of this technique is deliberated and evaluated on the standard IEEE 30-bus test system for different objectives. The obtained results of the proposed EEO algorithm is compared to the original EO algorithm and those obtained using other techniques mentioned in the literature. These Simulation results revealed that the proposed algorithm provides better optimized solutions than 20 published methods and results as well as the original EO algorithm. The EEO superiority was demonstrated through six different cases, that involved the minimization of different objectives: fuel cost, fuel cost with valve-point loading effect, emission, total active power losses, voltage deviation, and voltage instability. Also, the comparison results indicate that EEO algorithm can provide a robust, high-quality feasible solutions for different OPF problems.

The yellow rust of wheat (caused by Puccinia striiformis f. sp. tritici) is a devastating fungal infection that is responsible for significant wheat yield losses. The main challenge with the detection of this disease is that it can only be visually detected on the leaf surface between 7 and 10 days after infection, and by this point, counter measures such as the use of fungicides are generally less effective. The hypothesis of this study is to develop and use a compact electrochemical-based biosensor for the early detection of P. striiformis, thus enabling fast countermeasures to be taken. The biosensor that was developed consists of three layers. The first layer mimics the wheat leaf surface morphology. The second layer consists of a sucrose/agar mixture that acts as a substrate and contains a wheat-derived terpene volatile organic compound that stimulates the germination and growth of the spores of the yellow rust pathogen P. s. f. sp. tritici. The third layer consists of a nonenzymatic glucose sensor that produces a signal once invertase is produced by P. striiformis, which comes into contact with the second layer, thereby converting sucrose to glucose. The results show the proof that this innovative biosensor can enable the detection of yellow rust spores in 72 h.

The use of biocompatible and biodegradable porous scaffolds produced via additive manufacturing is one of the most common approaches in tissue engineering. The geometric design of tissue engineering scaffolds (e.g., pore size, pore shape, and pore distribution) has a significant impact on their biological behavior. Fluid flow dynamics are important for understanding blood flow through a porous structure, as they determine the transport of nutrients and oxygen to cells and the flushing of toxic waste. The aim of this study is to investigate the impact of the scaffold architecture, pore size and distribution on its biological performance using Computational Fluid Dynamics (CFD). Different blood flow velocities (BFV) induce wall shear stresses (WSS) on cells. WSS values above 30 mPa are detrimental to their growth. In this study, two scaffold designs were considered: rectangular scaffolds with uniform square pores (300, 350, and 450 µm), and anatomically designed circular scaffolds with a bone-like structure and pore size gradient (476-979 µm). The anatomically designed scaffolds provided the best fluid flow conditions, suggesting a 24.21% improvement in the biological performance compared to the rectangular scaffolds. The numerical observations are aligned with those of previously reported biological studies.

Nanoelectrochemistry allows for the investigation of the interaction of per- and polyfluoroalkyl substances (PFASs) with silver nanoparticles (AgNPs) and the elucidation of the binding behaviour of PFASs to nanoscale surfaces with high sensitivity. Mechanistic studies supported by single particle collision electrochemistry (SPCE), spectroscopic and density functional theory (DFT) calculations indicate the capability of polyfluorooctane sulfonic acid (PFOS), a representative PFAS, to selectively bind and induce aggregation of AgNPs. Single-particle measurements provide identification of the "discrete" AgNPs agglomeration (e.g. 2-3 NPs) formed through the inter-particles F-F interactions and the selective replacement of the citrate stabilizer by the sulfonate of the PFOS. Such interactions are characteristic only for long chain PFAS (-SO3 - ) providing a means to selectively identify these substances down to ppt levels. Measuring and understanding the interactions of PFAS at nanoscale surfaces are crucial for designing ultrasensitive methods for detection and for modelling and predicting their interaction in the environment.

Background: The coronavirus disease-2019 (COVID-19) pandemic has imposed an unprecedented strain on healthcare systems worldwide. In response, psychiatrist trainees were redeployed from their training sites to help manage patients with COVID-19. This study aimed to examine the attitude of psychiatrist trainees toward redeployment to COVID-19 sites and their perceived preparedness for managing physical health conditions during redeployment. Methods: A cross-sectional researcher-developed online survey was administered among psychiatrist trainees in May 2020 at the Department of Psychiatry, Hamad Medical Corporation, Qatar. Results: Of the 45 psychiatrist trainees, 40 (88.9%) responded to the survey. Most trainees reported being comfortable dealing with chronic medical conditions, but less so with acute life-threatening medical conditions. Half reported feeling anxious about redeployment, and most felt the need for additional training. We found that trainees' perceived redeployment preparedness was significantly associated with their level of postgraduate training and the time since and duration of their last medical or surgical training. Conclusion: Adequate preparation and training of psychiatrist trainees is important before redeployment to COVID-19 sites to ensure that they can effectively and safely manage patients with COVID-19.

The era dominated by the liberal world order, dating back to the end of World War II in 1945 and gaining unchallenged dominance with the fall of the Berlin Wall in 1989, is now coming to an end. Yet the universal principles that the era personified in its rhetoric, and in the best of its actions, lives on. The global community has a moral obligation to continue on its journey to build a more equitable, secure and prosperous world for all of its citizens and must devise investment strategies that enable progress to both endure and accelerate. "The Declaration on Science and the Use of Scientific Knowledge," issued at the conclusion of the inaugural World Conference on Science in 1999, contained a broad range of insights and recommendations that remain as significant today as they did 20 years ago. We would be wise to heed the challenges that the declaration conveyed by recognizing that the journey for equity in science, technology and innovation (STI) is by no means over and, in fact, remains as relevant now as it did then-both as an economic and social necessity and as a moral obligation. This paper seeks to explore how patterns of investment in STI have changed over the past 2 decades-and how they have not.

The use of external fixation devices is considered a valuable approach for the treatment of bone fractures, providing proper alignment to fractured fragments and maintaining fracture stability during the healing process. The need for external fixation devices has increased due to an aging population and increased trauma incidents. The design and fabrication of external fixations are major challenges since the shape and size of the defect vary, as well as the geometry of the human limb. This requires fully personalized external fixators to improve its fit and functionality. This paper presents a methodology to design personalized lightweight external fixator devices for additive manufacturing. This methodology comprises data acquisition, Computer tomography (CT) imaging analysis and processing, Computer Aided Design (CAD) modelling and two methods (imposed predefined patterns and topology optimization) to reduce the weight of the device. Finite element analysis with full factorial design of experiments were used to determine the optimal combination of designs (topology optimization and predefined patterns), materials (polylactic acid, acrylonitrile butadiene styrene, and polyamide) and thickness (3, 4, 5 and 6 mm) to maximize the strength and stiffness of the fixator, while minimizing its weight. The optimal parameters were found to correspond to an external fixator device optimized by topology optimization, made in polylactic acid with 4 mm thickness.

RESUMO

The detection of glucose is crucial in the management of diabetes and other medical conditions but also crucial in a wide range of industries such as food and beverages. The development of glucose sensors in the past century has allowed diabetic patients to effectively manage their disease and has saved lives. First-generation glucose sensors have considerable limitations in sensitivity and selectivity which has spurred the development of more advanced approaches for both the medical and industrial sectors. The wide range of application areas has resulted in a range of materials and fabrication techniques to produce novel glucose sensors that have higher sensitivity and selectivity, lower cost, and are simpler to use. A major focus has been on the development of enzymatic electrochemical sensors, typically using glucose oxidase. However, non-enzymatic approaches using direct electrochemistry of glucose on noble metals are now a viable approach in glucose biosensor design. This review discusses the mechanisms of electrochemical glucose sensing with a focus on the different generations of enzymatic-based sensors, their recent advances, and provides an overview of the next generation of non-enzymatic sensors. Advancements in manufacturing techniques and materials are key in propelling the field of glucose sensing, however, significant limitations remain which are highlighted in this review and requires addressing to obtain a more stable, sensitive, selective, cost efficient, and real-time glucose sensor.
